This should be a document/letter of your society that states that you are a representative of the society and that your participation secures endorsement of the update and revision of the Angioedema Guideline by the society you represent.

Conflict of Interest

In the interest of transparency, we ask you to disclose all relationships/activities/interests listed below that are related to your participation in the expert panel of the update and revision of the urticaria guideline. “Related” means any relation with for-profit or not-for-profit third parties whose interests may be affected by the content of the urticaria guideline. Disclosure represents a commitment to transparency and does not necessarily indicate a bias. If you are in doubt about whether to list a relationship/activity/interest, it is preferable that you do so. All declarations of potential or actual conflicts of interest, whether due to a financial or other relationship, must be provided to the Global Allergy and Asthma Excellence Network upon submission of the application. Declarations must include whether any fee, honorarium, or arrangement for re-imbursement of expenses in relation to the update and revision of the angioedema guideline has been provided.
