This project is still actively recruiting participants for this study. The deadline was extended to  July 31st, 2023. Your participation is essential to the success of this project, and we appreciate your ongoing commitment.

Please kindly note that the survey consists of two distinct options: the patient’s survey and the doctor’s survey. Therefore, if you have already completed the patient’s survey, please refresh the page and select the option for the doctor’s survey in order to complete that portion of the survey as well.

Furthermore, it is important to ensure that the same code is used in both the patient’s and doctor’s survey forms. This is necessary in order to accurately match the information provided during analysis. Thank you for your cooperation.
